Crohn's Disease - Update on Zach

We are in the emergency room at The University of Alabama at Birmingham.  Zach started passing blood in his stool.  We have been here since 7:12 P.M. on 5/28/2010.  It is now 4:47 A.M. on 5/29/2010.  They have done some x-rays and bloodwork.  We still do not know what is going on.  If you can help with medical bills, please go to "Wish Upon A Hero" website to make post.

Hi, I'm Aurora. I'm actually an Aplastic Anemia Survivor, (sever aplastic anemia survvor.) I can only give my hope for you and your family. I've no money to give, but hope is stronger than that (go to my aidpage area and see what I mean.)

 Aplastic Anemia has a 80% cure rate, her chances are very good.

Stay Strong. You may or may not be Catholic but still.

Child with Severe Aplastic Anemia needs help

Hi.  I'm here to ask for help on behalf of my 17 year old niece, Taylor. She lives in Ohio with her mom, dad and younger sister. Up to a few months ago, she was a senior in high school, planning for prom and graduation, looking for a job and truly enjoying  her life.  She started getting sores on her legs and body and noticed she was bruising very easily.  So her mom took her to the Dr., who ran some blood tests on her.  The next day they received a phone call telling them to get her to the emergency room right away.  Her red and white blood cells were unbelievably low.  After more testing they discovered she has a terrible life threatening disease called Severe Aplastic Anemia.  She started getting blood transfusions twice a week to keep her counts in a reasonable range and she was also put on several medicines.  Any infection could be life threatening.  Her mom, dad and 5 year old sister were tested for a bone marrow match, and none will work. Financial Assistance Policies:

The Jennifer Ashleigh Children's Charity assists children who are seriously ill, have a permanent disability, are 21 years of age or under and whose permanent residence is in Ontario. Family income impacts on assistance decisions.

We will consider requests for assistance in the following areas as they are related to the child's illness:

  • Emergency financial relief
  • Respite for a pre-determined period of time
  • Developmental therapies (excluding Hyperbaric Oxygen, ABA, IBI or RDI therapy)
  • Educational programs, materials, instruction
  • Specially adapted computer equipment and software
  • Medical treatments not covered by government health plans or insurance
  • Recreation that promotes a child's involvement in the community.

To Request Assistance:

Please call the Charity office at (905) 852-1799 and ask for a Request for Assistance form. The form may be completed by the child's parent, physician, therapist, social worker, teacher or community liaison.

Commonly Asked Questions/Answers:

My child has Cerebral Palsy. It's not a serious illness. Will you still consider assisting us?

Yes, we do consider assistance to children with permanent disabilities under our guidelines.

How soon can I expect a call back after leaving a message?

Please understand we receive more than 30 calls a day from parents making inquires on the status of their application, as well as a high number of applications. We will respond as quickly as we can however you may not receive return call for 3 business days.

How long will it take to process my application?

Our first priority as an organization is emergency assistance. All other applications are reviewed as soon as possible on a first come first served basis.

Does Jennifer Ashleigh Children's Charity have an income guideline and what do I need to provide?

Yes, in order to help those with the greatest financial needs, we will only consider requests from families with a combined annual income of $60,000.00 gross (before taxes) or less. Please provide a copy of your current Income Tax Notice of Assessment. For example, we are now in the year 2008 - we would require proof of 2007 income.

The Jennifer Ashleigh Children's Charity application states proof of income must be provided. I am self -employed and do not file my taxes until later in the year. What do I do to meet your requirements?

If you are self-employed, we require the Statement of Business Activities (T2124) that is filed as part of your T1 General tax return. As we are in 2008, we would request this information for the tax year 2007. If your request is for emergency funding and you have not filed your income tax yet, we would request that you provide information for 2006 and the Advisory Board will review your request on an exception basis. If the request is not urgent, we would request that you wait until your income tax is filed for the current year.

Our income is over $60,000 combined but we have more than one child with special needs. Can we submit an application for our children?

Our Advisory Board will consider assistance if you have more than one child with special needs and your expenses associated with these children are exceptionally high.

Can you help me with purchasing a computer for my child?

Assistance will be considered only if your child requires specially adapted computer hardware as recommended by a medical professional. If your child can use a regular computer we do not assist. We will consider assistance with specialized computer software if recommended by a medical professional or your child’s teacher.

Do you help with mobility equipment such as a walker?

The only mobility equipment that we assist with is a specialized trike if it is recommended by a medical professional.

You have assisted our family in the past, are we able to apply again?

Yes, you are able to apply to JACC again if your child’s expenses due to their medical needs are still over and above what your financial situation can cover. Please note that we are receiving a high number of requests and we will do our best to assist where we can however we cannot guarantee ongoing assistance. Please ensure that you continue to look for alternate organizations, service clubs etc. for additional assistance.

The Jennifer Ashleigh Children's Charity assists with emergency household expenses. I’ve recently lost my job; can you help me with my mortgage?

Our guidelines with regard to emergency household assistance are to assist when expenses due to a child’s medical condition cause the family to be unable to pay their bills. Unfortunately if the need for financial assistance is not due to the child’s expenses, we do not assist.

I need to fence my yard for my child’s safety, can you help?

We are unable to assist with home and property renovations under our guidelines.

We need a modified vehicle, can you help us?

No, the charity does not assist with vehicle purchases or modifications.

Does your organization help with camp?

It has been our policy to fund only when no other assistance is available. We do not fund where other funds are available and/or when the camp will absorb or assist with any funding. E.g. - if a child is going to an Easter Seal camp, we would not assist with another camp request. Our maximum assistance considered for camp would be for one week only during a calendar year. Our first priority as an organization is to assist with emergency funding. We do not assist with sibling camp fees. Worker assistance for camp would follow the above guidelines as well.

We need assistance with our child’s therapy, preschool fees and transportation to appointments. Can you help me with everything we need?

Please prioritize your needs on your application. Due to the high number of requests we receive, we are not always in a financial position to assist with everything requested. We will give consideration to your greatest needs first.

What kind of therapy do you cover?

Our organization will consider assistance with traditional proven therapies such as speech therapy, physiotherapy and occupational therapy. Written documentation from your medical doctor recommending therapy for your child must be provided with your application. Please note hyperbaric oxygen, RDI, IBI, and ABA is not supported. We do not support any services or products associated with these therapies including transportation, assessments, materials or direct therapies.

Do you assist with respite care?

If the child’s medical requirements warrant more respite care than offered through Special Services At Home (SSAH), Assistance for Children with Severe Disabilities (ACSD) or the family is on a waitlist for respite care, we would consider assistance. We would require a written recommendation from a medical professional or social worker familiar with your situation and needs.

A family member is providing care for our child and driving them to school and medical appointments. Do you provide financial assistance for payment to a family member?

No, we do not reimburse family members as our assistance is intended to provide additional support beyond that which is normal family responsibility.

Do you assist with fees for community recreational programs?

If the recreational program is recommended by a medical professional to assist with the child’s development or social integration into the community we will consider assistance.

Do you call me when the assistance provided is complete?

No. It is the responsibility of the parent/guardian to track all funds utilized/paid to service providers.

We have helped more than 1,000 families including our first international family who had found us on the Internet. The amount of our grants are growing in proportion to the monies coming in to Aubrey's Foundation. We are pleased to say that we are able to help more families with bigger amounts than we've been able to in the past.

The Aubrey Rose Hollenkamp Foundation will award financial help with a family's medical bills for their child who has a life-threatening condition. As our funds grow, so will the number and the amount of the grant that will be awarded.

Preparing Your Grant Application

The trustees of the Aubrey Rose Hollenkamp Foundation want to give your proposal the consideration it deserves. your grant application is the most important stage in the Foundation's evaluation of your proposal. For this reason, you should address each question asked, specifically and completely.

The first step in the application process is your "Letter of Inquiry to Apply". This letter, listing some of the basic information regarding your situation, will assist our Trustees in determining if your request for a grant will be a match with our mission in the community. Some, but not all inclusive, of the initial information will be asked to provide are:

  • Tell us about your family and where you live
  • Description of your child's situation
  • Objective for what you are asking for -
  • Explanation of benefits statement and Coordination of benefits statement (if applicable) from insurance company(s). Along with copies of actual bills that you want us to pay

 

Submitting Your Application

You may submit your application at any time during the calendar year. The Foundation's Board of Trustees meets quarterly to review grant applications. Applicants will be notified no later than 60 days after review. Applications may be mailed to:

Eligibility

Grants are awarded based on need. If a family has outstanding medical bills that insurance will not cover, our Foundation can possibly help out a family in need until our annual funds have been exhausted.  As our funds grow, so will the number and the amount of help we will be able to give.  We appreciate families just asking for one grant per family so that we can help as many families as possible.

 

Ineligible Requests


Ineligible requests such as medical bills already paid, submitting for food, clothing, laundry fees, mortgage payments and associated homeowner bills and anything deemed non-medical for your child will not be acceptable criteria to submit a grant request.

 

Our Guiding Principles

Our philosophy is to provide a balanced giving program that considers the total well-being of the child and how the request benefits their family also. Aubrey Rose Hollenkamp was always a very happy baby throughout everything she endured including a heart and double lung transplant. She smiled continuously throughout her short life. We would like to carry on her spirit by being able to put smiles on other children's faces and their families. We can do this by helping to pay some medical bills that normally wouldn't be covered.

  1. Please tell us in a concise manner about your sick child and his/her condition and prognosis. 
  2. Please tell us about your immediate family.  Please provide mothers full name, father’s full name with first and last names and ages of children in family.  If parents live in separate homes, please state full information for both.
  3. Please attach the explanation of benefits from your insurance carrier and also, attach the coordination of benefits statement from your secondary insurance, if applicable. Also attach any bills you would like to have paid.  Copies of bills are acceptable as long as they are legible.
  4. Explain what other related bills that you have because of your child being sick (for example, Ronald McDonald House expenses for out of town medical treatment).
  5. Explain what you would like to have paid and who that payment should be made payable to.   Please provide an itemized page with the name of the organization to be paid, their telephone # with area code, account # of claim, date of service and amount to be paid.  Bills will not be paid for without this itemized statement.
  6. Help with medical bills

  Hi,  My story is one of many I am sure.My husband is disabled, we have 4 children. We have been doing everything the Social security office wants us to do but ,we are in appeal again.This has been going on since 2004. I have been trying to maintain my family and my bills.We have one bill to our Family Dr. that is rather large in my standards. It is now to the point were my husband might be told he is  no longer a patient of our Dr. She has been our Dr. for 8 years. She has been  with us through all of this. She would be happy to just make the bill go away but ,the hospital she is working with won't. We live in a 800 sq foot home we rent. My husband and I have made our bedroom in the kitchen, I should say our bed is in the kitchen in the corner. Two of our children live with my husbands parents.Two of our children live with us. My income is not enough to pay rent, lights ,phone and buy food never mind keep the car running to get me to work. I am a teller in a bank, In Connecticut a teller makes about 10.00 a hour. Taxes and etc. I bring home approx  1450. dollars a month. My rent is 1050.  Leaving me 400. dollars to pay all other house expenses. In Connecticut you can only get welfare for so long. We used that up.The Dr. bill is about 2400. dollars. as you see I can not pay that.  If some one knows how I can get it paid please tell me. Any and all suggestions are helpful. Teenage Daughter Needs $$$ for Weight Loss Surgery

We've exhausted all of our financial resources for my 16-year-old daughter who has been dealing with obesity for 8 years. Our family moved out of our home into an apartment and have sold most of our furniture to stay afloat after my husband experienced a heart attack last year at the age of 49. Because of medical bills for both daughter and father, basic expenses such as rent, utilities, and groceries are not being met. My daughter is missing most school days because of the intense loneliness and embarrassment she feels. Many of her longtime friends have abandoned her because she no longer "fits in" with the thin crowd. She is the most gentle-hearted soul and it breaks my heart to see her wall herself in from her peers. She used to have a healthy sense of who she was, socially active, and an honor roll student who was always instrumental in bringing friends together. Our finances no longer allow us the priviledge to provide counseling for her. She has attended five weight management programs designed for children and teens, but she has only gained more weight. Her health issues are: pre-diabetic, high blood pressure, polycystic ovary syndrome, and pre-disposed to heart disease. 

During the summer, she asked about bariatric surgery as an option.  She spoke with a surgeon about laparoscopic surgery for two hours and was so excited from what she heard, that she told all her friends and family about her upcoming surgery. This is a huge financial endeavor since our income cannot pay the needed $5,500 that is required before surgery is scheduled.

Her emotional turmoil was so intense last night that I asked her if she could have one wish, what would it be? "To be thinner," she said.

Please, can anyone help her with her ONE wish that could literally give her longer life physically and emotionally?

Overwhelmed with Medical

I am a mother of two who have grown up to be beautiful children.  They 2005 year was one tragedy after another in our home.  My husband had an emergency triple by-pass heart surgery.  We almost lost him twice.  As this was occuring, we discovered that our son had a spot on his brain.  After going to many clinics in MN, he was diagnosed with a brain tumor.  He needs to have more tests, but is afraid and keeps putting it off.  My husband was laid up three months due to open heart surgery.  Our son has severe headaches that pretty much lays him up.   We discovered that our daughter was expecting a baby; she is also a full time student going for her nursing degree.  We help her out as much as we can.  This fall, my husband ended up in treatment for alcohol abuse.  He was at the point of no return.  We need help, on top of all of this we almost lost our home due to foreclosure.  I don't know where to turn now.  We are so far behind on our bills and we can't afford the medications that my husband needs.  Our prescriptions went from $60.00 per month to over $400.00.  I suffer from severe depression and do not work. 

I hope there is someone out there that can answer our prayers.  Things are just piling up and we can barely reach the top.  Someone please help our family.
